To start the process, all you have to do is call or email our clinic at (918) 664-3571 and set up a time with one of our staff. They will be happy to assist you in finding the best time to schedule your first appointment.

To save time during your first appointment, please print and complete the following forms. Please sign and date them with the date of your first appointment.

- New Patient Paperwork

- Auto Accident Paperwork (fill out along with New Patient Paperwork only if it applies to you)

In addition to the paperwork, please bring the following on the first visit:

- Photo ID
- Insurance card or information (if applicable)
